What OS are you on? On Windows, NVDA (http://www.nvda-project.org) does what you want, and is free. On OS X and IOS, Voiceover is built-in to the OS and does these things.

Well, some of it is actually just software. On Windows, I use NVDA. Some prefer JAWS or Windows Eyes. On OSX, Voiceover is built in. The majority of blind people use iPhones, because Voiceover works really well compared to other competitors. As far as text to brail, yes it's called a braille display. I really don't use them, but I believe the simple ones you just plug in and the text will pop up on the device.

Look into braille displays and depending on your needs into braille notetakers which can be described as braille PDAs.
