My mom is blind in her left eye and severely impaired in her right. (self.Blind)

She is having issues being able to see and read her phone. I have no idea how to even begin to help her! What apps / settings for a galaxy s5 can I put on her phone to help her use her phone better?

Check out the options beneath Settings > Accessibility. Android has $1 to increase font size and contrast and to $1. A few apps I find useful:
* $1, if she'd like to use a screen reader - $1
* $1, for magnification
* $1, for OCR
* $1 and $1, for e-books.

Most blind people use iPhones for a reason. The screen reader and magnifier on iOS are better. They're easier to learn and easier to use. There are many more apps for blind people on iOS than Android.

If I were in your shoes, I'd sell the S5 and get an iphone for her. Assuming she has no interest in tinkering around with her phone and just wants to be able to use it, an iPhone will serve her better.

I will let her know this. I'm sure her upgrade is up because she has had her phone for a few years now. Is it just any iPhone or do you suggest a specific one?

Any of the current models would be fine. The SE, 6s, or 7 would be fine with Voiceover. The 6s Plus or 7 Plus would be better if using the magnifier feature because you can fit more text on the screen.

I would just keep in mind that even if she gets the Plus, she will probably want to learn how to use Voiceover as well. Text to speech is much faster to use than magnification once you get the hang of it, and very useful for reading emails, articles, and other long blocks of text compared to the magnifier. The magnifier is good for things like looking at pictures, navigating graphical or inaccessible apps, and so on.
