The term sighted guide refers to a specific method of guiding someone (someone sighted guiding someone who is blind). For question 7, the question would be better phrased "What method do you use to travel around?". And as /u/fastfinge said, the proper term is a cane (to be specific, a mobility cane).
Overall, your survey uses the phrase "sighted guide" as a catchall term for travelling independently.

The term "blind stick" is not correct; the word you want is cane. Also, question 8 is unclear/inaccessible. Does 1 star mean most challenging, or is 5 stars most challenging. I suspect this is indicated visually, but not to screen readers.
