Research surveys must include ethical research disclosures; who is asking, how will the information be used, how will personal privacy be maintained. You may also have better luck just asking the same questions as a post.

The survey doesn't identify who is asking, how the information will be used or how personal privacy will be protected. It will not be approved until fixed.
