There was a wave of blind-friendly IRC clients 10, 15 years ago. I think they started with Accessible Chat from gamesForTheBlind.com. It was a great community until it imploded.
Before that and carrying on for a time afterward, of course there was a huge IM presence. MSN/Windows Messenger, AIM and ICQ, blind people were heavily engaged on the platforms.
Then things seemed to fragment. IRC is still going, but it's not grown in popularity and I think the people who used the accessible clients moved on. Skype and TeamTalk ar still used for small groups, and some people use Discord as we've already said.
BCAB hold online events momthly focused on technology, and VITalk on facebook do FB messenger chats.
It's an interesting landscape, the history of which is also fascinating. I am running a MOO server where I teach object oriented programming, and even after 1 class, people are hanging around just chatting. So the spirit is certainly there. :)

Discord is also a chat platform, that can be used on your computer or on a smartphone. I don't believe it has the best support for screen readers, from what people have said.
