I'm technical so I host my own remote server and have port 6837 open on both my router and firewall.

You need to set up NVDA Remote on both systems. Now, there are some decisions you can make about this, depending how technical you are.
If you can open the relevant port on your router at home, you're better off doing that, as it's faster than using a server. If you can't do that, you need to use an NVDA Remote server. You can run one of your own on a dedicated server or use someone else's.
On the computer you want to control, you go to the NVDA menu, tools, remote, options, and choose autoconnect to server on startup. If you're opening a hole on the firewall, choose host control server, otherwise, use external server. Finally, choose allow this computer to be controlled, and a password, which you can have generated randomly. You will need to remember or take note of this password to control your computer. If anyone has this password they can control your computer and your whole security is shot.
On the laptop, go to NVDA menu, tools, remote, connect. From there, choose client, control another machine, on host put the external IP of your computer, or the server hostname, and then enter the password.
