Survey Monkey and Google Forms are quite good and have information on their websites about creating accessible surveys. Qualtrics and Survey Gizmo have problems with some assistive technology configurations.

Google Forms is a good choice. I haven't used it a lot, but it's quite accessible. Plus, everyone is using it.

Yeah, Google Forms is 100% accessible.
