The Unicode consortium - who agrees the emoji standards - also agrees on descriptions of the emoji. It is these descriptors that are announced by screen readers.

As an addendum, a lot of sighted people use emoji in contexts contrary to their description.

Now I see what you mean. Apparently, I have been doing the same thing without knowing. Oops.

similar to how people use sarcasm in language. It's a linguistic convention as much as it's based in Emoji, isn't it?

So wait. That emoji means tears of joy?! I thought it was crying laughing. Oh wow. I need a screen reader.

To answer the question, I only use the same 5-6 that I can see/interpret what they mean. The others are either too similar to another, I don't understand what they mean, or the proper context to use them doesn't come up in regular conversation.
