Online sleep survey help (self.Blind)

Hi everyone,
We're currently running an Online Sleep Survey to understand more about the association between vision impairment and sleep, health, and wellbeing. We've recently made some changes to our survey to try and make it as accessible as possible for visually impaired individuals and we're wondering if anyone who uses a screen reader could go through the survey and let us know if there are any issues? The survey takes about 45 minutes to complete.

I have to agree, I would do a 10 or even 20 minute survey for free no problem but this is a lot of time.
