With the new iOS that just came out it really whacked out the punctuation settings I had made, along with the activities that triggered them. I don’t know if it has anything to do with emoji‘s but food for thought

Check that the setting to read emojis is set correctly. You may need to disable re-enable it.

if that meets with no success, try a different speech language or voice and check you're not using screen recognition or similar.
If this is app specific it may of course be some rendering within that app for some reason: unusual but certainly possible.
