how to ophamologists measure visual acuity when you can no longer see the chart at 3 feet?(self.Blind)

My exams mostly consist of large letters on a digital screen. I'm guessing they can make an estimate from the size of the letter used. I've also been tested with large letters on a flashcard & the distance needed for me to identify the letter.

Counting fingers, hand movements, light and shadow, I think. I don't see the chart out of my left eye, and that's how they "test" the state of that eye.

how is counting fingers accurate as there are so many variables to take account of and what does that count to numerically?

It isn't particularly accurate, but they sometimes add notations that indicate at what distance. However, if someone's vision is so poor they are counting fingers, it doesn't have to be exact.
