Annotating with a screen reader (self.Blind)

Just wondering if anyone had any experience or ideas on annotating with a screen Reader.

I'd love a tool that let me import a screenshot and, a bit like how a screen reader interprets a powerpoint slide, describes the things on the image and their relative positions in a linear, tab-around approach. I'd like to be able to highlight elements or parts of them (such as specific bits of text) and apply effects (circle something, change its colour, redact, etc). Or even just a snipping tool that let me know verbally or in Braille what I'd snipped. My screenshots are currently an all-or-nothing approach as far as a window on my screen is concerned.

Never found a way to do that, but it would be nice.
