I can only comment from a US perspective that resides in the state of Pennsylvania. I'm sure our federal laws about discrimination are different as well as labor related required accommodations. Hopefully there is some type of province or city based group that assists the disabled in getting and staying employed. Contact them and ask for assistance as they will be current with all the requirements and work for your rights.

Also in Ontario here. Is the HR department the one requesting all this documentation? Usually that documentation you provided is sufficient, not sure why they need to go through SunLife (unless it is for funding for something specific).

You may want to talk to ARCH Disability Law Centre about it to check up on the legality. There may be legitimate reasons, but there may also be nefarious reasons.
Also, your work legally cannot require you to use your ADP funding to get equipment for work.

"Eligibility includes any Ontario resident who has a valid Ontario Health card issued in their name and has a physical disability of six months or longer. **Equipment cannot be required exclusively for sports, work or school**. ADP does not pay for equipment available under the Workplace Safety and Insurance Board or to Group “A” veterans for their pensioned benefits. There are specific eligibility criteria which apply to each device category."
You can get it, and **choose** to use it for work, but work itself can't require it.
