So I finally got told what the determination of my low vision assessment was, and I went and went over the information regarding how the scale works but it's still confusing. Anyone better able to explain what the different categories mean? My left eye is a four, my right eye is a five.
Edit: so I was able to find what the scale/categorization they are using is, it's the ICD-10 scale that is I guess used by most places. Only real difference it seems between a four and five on that scale is that my left eye has light perception and my right eye is totally blind no light perception.

Yeah, it's the ICD-10 scale (which is usually used in health care, but not often given to clients/patients. Even the eye doctors don't usually put that into the eye report, they usually report visual acuity). It is sometimes called Functional Vision Codes as well (that is what we call it in Canada).
Visual Fields
2 means 20 degrees to 11 degrees
3 means 10 degrees to 6 degrees
4 means 5 degrees to 1 degrees
5 means No Light Perception
Visual Acuity with correction
0 means 20/20 (6/6) to 20/69 (6/17)
1 means 20/70 (6/18) to 20/199 (6/59)
2 means 20/200 (6/60) to 20/399 (3/59)
3 means 20/400 (3/60) to 20/1199 (1/59)
4 means 20/1200 (1/60) to Light Perception of < 3 feet
5 means No Light Perception
9 means UNDETERMINED or UNSPECIFIED (For use with children or profound developmental delay)

Is this coming from a doctor's report or some government agency report?
What country are you in?
What type of report is this? Does it have a specific name?
Does the scale have a name? (i.e. Bristol Stool Scale)

I'm in the US, it was a report from my state's commission for the blind from the low vision ophthalmologist they sent me to, it seems to be the category of visual impairment scale, not sure if it has some other name.

I’m just guessing here but could the numbers represent your degree of visual field in each eye?

I don't think so, I know that at least in my right side that is listed as five that that seems to indicate total blind and LP. So I think the scale goes from 1 to 5.

I know that the other eye is light perception, minimal distance find detail movement etc.
