JAWS and NVDA include OCR capabilities. JAWS allows you to save the recognized text as a Word document, and if you're using NVDA, you can use something like Robobraille to OCR the document into an accessible file.

I haven’t found a good program besides Adobe Acrobat for retagging a pdf for accessibility. The free programs (on Windows, at least) haven’t been useable when I have tried them.
