You performed an in place upgrade from Windows 10 to Windows 11 and NVDA was installed prior to the update? The operating system update will have added and changed a number of devices and really messed with the COM registrations that NVDA relies on. You might want to try running the NVDA COM Registration Fixing Tool. You can find it under Tools in the NVDA menu.

Have you tried a clean version, uninstall and reinstall type of thing?

I’ve not had that but I’ve had it not read the start menu, I had to restart it.
Are you using Bluetooth headphones?

I can certainly relate to the file explorer thing. Unfortunately, at least for me, this is a long-standing problem that spans many versions of both NVDA and Windows, including NVDA 2017.3 on Windows XP! I haven't figured out a way to work around this issue, other than completely restarting the OS, which is a pain in the backside, especially if I'm working on multiple projects at once.
