While fmla probably isnt an option, you can request short term disability if you have it. I would look for a doc that could do the surgery sooner than later, until then you may need acommodations at work, an use public transport or uber. It will be hard but you can do it!

First things, get on short term disability and FMLA. FMLA should last you 6 months, while short term length depends on company policies. FMLA will ensure your job and insurance is retained while short term puts money in your pocket, but may not last as long. You will need a letter from the doctor which explains things to HR.

Unfortunately they just started the job, fmla wont kick in until you have been there a year in most cases. Short term disability should cover it though, if they have it. America is hard for us
