When you press windows key, the start menu should open, unless you have changed some settings or installed some start menu tweek thing from the internet.
When start menu opens, the curser is placed in the search box, typing anything will cause search results to showing up. Pressing down arrow places you in the pinned apps section, which is grid of icons, you can customize them to your needs. While focus is on any of the pinned icons, you can press alt+shift and arrow keys to move them around in this area. Pressing up arrow after opening start menu will move focus to user account button, and around that you will find your pinned folders or whatever. That area also has the power option, which contains your shutdown and restart options.
Pressing tab from that search box, without typing anything also moves you through the sections of the start menu. By pressing tab, you can find all apps button, which opens up a list of all installed apps, softwares, android apps, everything on your system.
If this behaviour is not observed, perhaps contact microsoft disability support, or reinstall windows.
Another thing might be potential cause of this issue could be your NVDA version. Not sure if older versions of NVDA will support Windows 11, I never tried. Updating it might help.

No. I can only navigate in the desktop bar (don't know the exact name of the thing that have the clock / windows button)

what exactly do you mean by not being able to navigate to the start menu? can you elaborate on this a little, e.g. what keys you're trying to use to navigate or how exactly you are attempting this?
the windows key on your keyboard should always open the start menu, regardless of what screen reader you're using. same with windows + m key to minimize everything and go to the desktop, or windows key + b to go to the taskbar.
from the task bar or desktop, you can use the tab key to navigate through the different elements to get to where you're trying to go to
