Just tell the truth. Your (potential) new sm will call prsc to verify re-hire eligibility and will be told the reason for separation and the notes (if any) that we're recorded by the separating manager. So better to be upfront and honest about the circumstances surrounding it then to have something come back and bite you in the ass.
