Ask around your coworkers, you may have some who are looking for extra shifts and you can pawn off a couple of shifts on them. Keep an ear out for someone who is getting a new apartment or starting school or seems like they have some big holiday/vacation plans...

i had the same issue a few weeks back! i’m a full time student and i was getting scheduled for over 35 hours a week. one thing my coworkers said to try was to first say something to the manager, which for me worked bc she’s good and wants us to be happy and not burnt out. second, the other thing they said to try was to make your availability specifically the amount of hours and the times/days you want to work so they can’t help but schedule you for it! i’m really sorry this is happening, it sucks big time. but with the help of your partners you can do it! good luck!!

I hope you don't have to feel scared to discuss your availability with your manager with the sole intention of compromising in order to find what works best for you. Especially with your disability, there should absolutely be no suspicion of retaliation occuring for communicating your maximum hours per week, as you have had a valid reason from the start.
In the mean time since the schedule is typically made 3 weeks ahead, I would try to call surrounding stores to ask if anybody wants shifts, even if it's just one or two within two weeks. The sooner you make those calls, the greater your chances for success will be, especially with the ongoing labor hours being cut and the holidays coming up, you should have no problem with your store employed partners as well as nearby store partners taking 2 to 3 shifts off your hands.

I am part time so I set my maximum hours to 30 and my min to 20. I pretty consistently work 25 hours a week which is good. But I had to talk to my manager about it a few times.
Hopefully you can share what you said here with your boss. If you mention you like working but are concerned with burnout, they should be good with helping you without being a dick about it and underscheduling you

You can always see if other stores in your district have partners who would pick up some of your hours too!!!
