Medical LOA will require a doctor be involved. Sedgwick is super easy to deal with and you can pretty much complete the whole thing online. A doctor will have to diagnose and give a medical reason and length of time for being out of work. Sedgwick will contact the treating physician for you though, you just have to provide contact info.
