Something that can help with the pour - grab the cup and tilt it about 15-25° and start your pour down the side of the cup, slowly. When it gets about 1/2 to 3/4 full, slowly straighten the cup and move the pour to the center of the liquid. As you complete the pour it should naturally begin to make the white dot!
Then you want to be careful with your steaming. I started with the steam wand at the bottom, then slowly bring it up. You want the paper tearing sound for about 1 second, but watch your milk and make sure you make a whirlpool with steaming! It should help get rid of those bubbles.
You're making great progress!! It took me months to find the sweet spot when steaming and make a nice flat white :)
