Please put yourself first! This is a coffee shop job. Use the benefits available to you and do not feel an ounce of remorse for setting yourself up for success. You have two trained professionals telling you a very good idea.
As far as repercussions are concerned, I'm not at your store. I know how I'd react, and how my SM would react, but unfortunately not every boss is kind. Most likely worst case scenario is your request is denied and you keep working lots. Most likely best case scenario is you get those perfect 4 hour shifts. The Actual Most Likely Scenario will probably be somewhere in the middle.
But please please plz put yourself first now matter what. The world only gets one of you.

Always put your health first. You cannot get it back later, no matter how hard you try. Try not to get into the habit of ignoring medical advice. Also, don’t worry too much about what others think, including bosses!
You don’t have to give a reason and I would not. Just tell him your availability. You’re not require to disclose a medical condition and they are supposed to give you reasonable accommodation if you’re ill.

Put yourself first. I would talk to your SM as well as maybe put in a request for an accommodation.
Honestly I would just say to your SM “Hey I wanted to talk to you about my schedule. I’ve been struggling with some things and want to talk to you about maybe shortening some of my hours. It’s been suggested by my Lyra coach that I talk to you about it.” They don’t have to know about the program or mental health issues. They can’t fire you because you have a lentils illness. As long as you’re still available during the times they need I don’t see the problem.

I struggle with mental health issues as well. I can only say two things, one being that your supervisor isn’t that one that’s in charge of your schedule. Speak to your SM or ASM and communicate with them. The other is anecdotal and that I’m going based off of what you said here, do what your counselor recommends. It’s gonna be real hard to see any mental health improvement without making any change to your routines. If anything, by speaking to your SM or ASM, yall can work something out, but you have to communicate with them. Btw I’m just a public health major working in my local Starbucks, this isn’t any professional advice, what you end up doing is up to you. Best of luck!
Edit: also, don’t break your back for this company.
