From my understanding, you're getting 5% or $12/hr, whichever is more.
If you make $11 now, a 5% bump would be $11.55 -- you'll go up to $12.
If you make $11.50, a 5% bump would be $12.08 -- that's your rate.
If you make $12, a 5% bump would be $12.60 -- that's your rate.
If you've been a partner for 3+ years, it's 6% instead of 5%. And certain markets are getting an extra bump in addition to the nationwide raise, to bring them more in line with other jobs in the area so Starbucks can stay competitive.
All this speculation doesn't ultimately matter, because your raise will go into effect on October 1 and then you'll know for sure what your new rate is.

If you’re making under $12/hr you’ll be bumped to 12. If you are making 12/hr already and you’ve been there less than 5 years ( I think. Either 3 or 5) you get a 5% raise if you’ve been there more than that it’s 6%.
