Lol this has happened before, it's always just reapply and resubmit. It's annoying but it works. SOMETIMES they can revert to previous step and resubmit it, but that rarely works. Glad you got it fixed though!

your manager can also go into the recruiting system and revert the last step. basically they revert the offer and then submit the offer again. happened to me this last week with a new hire and this fixed it right up. :)
