A customer ordered a SCAL and requested soy milk instead of almond. During this rush I was on bar and was making the drinks and genuinely thought I had put soy milk. Mind you the customer had not told us she was allergic to almonds. :’( So I sent the drinks out and that was that. A few hours later she calls my store and cusses my co worker out saying that we lied about what milk we had put into her drink. I feel terrible and will of course be more aware of what is requested in the drink. Just wondering if this has ever happened to anyone else.

Yes. I've made that mistake before. I've accidentally used non-decaf in decaf drinks before. I always remake it if I remember, but sometimes shit happens and you remember when you get home.
We're humans. There's hellish lines. Sometimes milks get mixed up, and we are simply too stressed and don't recognize our mistakes until we actually get off autopilot mode. We try our absolute best and shit happens. To say that a barista has never mistakenly put in the wrong milk, foam, or syrup is to say that we are simply robots. It's horrible that we make these mistakes.
HOWEVER it is policy to make sure they know that mistakes CAN happen, and that cross contamination is very likely. If you're allergic to something, TELL the barista to especially be careful of it.
She should not order a drink that ESPECIALLY comes with a nut milk if she's allergic to nuts. That's a nightmare waiting to happen.

I've had alot of customers just complain we put the wrong milk in even when I know for a fact that I did the right milk in some like to complain alot espically with the brown sugar shaken oatmilk espresso people always says it taste like almond milk not oat milk so honestly it's not your fault even if u did make a mistake we all make mistakes and they can't expect us to be completely perfect for how little they pay us
