Your manager should communicate any issues with you. Assuming you have a good manager you will be talked to and coached on what you need to improve on. If there is an on going issue you should be talked to, warned about a possible write up, and then written up and talked to again if the issue continues.

When you sign the write up, you are agreeing that your manager and you had a discussion about you being written up. You are not signing that you agree with it, just that you are aware that you were written up
