There are multiple. It all depends on which one at which level you choose but the main carriers that I know of are Aetna, Premera, Cigna, Athem, United Health, Blue Cross… depends on the state you live in. From the they have levels of bronze, silver, and gold. I would call the pcc to find out more info. Enrollment for my area is next month so I know info is being sent out in a couple of weeks.

Directly related to Texas and top surgery, most of the providers are covered by the American Institute of Plastic Surgery but with requirements. I personally chose Aetna the first time around. And i think I’m now with Premera Blue Cross. Last time i checked for a consult with Dr. Raphael, a full continuous year of Therapy accompanied with a letter from them, was a requirement in order to use insurance. Then from there it’s used just as any other insurance, they cover a percentage and you are responsible for the rest. I found that when first advertised Starbucks’s made the insurance seem that Top Surgery was free through their insurance, but what they were actually advertising was an insurance plan that accepts and covers Top Surgery. But so do many other insurances.
