It simply means you most likely broke some rule or policy, and your SM is documenting that they talked to you about it, informing you of the infraction and giving you the opportunity to learn from it and grow.

Ohhh okay, that makes me feel better. I got written up because my shoes were out of dress code. I’d been wearing them for months and I thought they were approved but I guess not 😔

dunno if it's changed, but if you get three they can then fire you at any time they want. they are permanent
iirc the person being written up also has to sign it, so if someone believes they are being written up unfairly they can do something about it.
