I know the line to go to for various situations within the store: Shift, SM, then DM. But the situation that I’m wanting to write a formal complaint about involves all three of these lines. Who would I go to above that, if at all?

I would either reach out to the Partner Resource Manager for your district or call Ethics & Compliance like others have suggested.
If you don’t know who your PRM is you should be able to find that out by calling the partner contact center. They’ll either give you their info or set something up for the PRM to reach out to you.
