I work about 35-40 hours a week and it’s starting to catch up with me. I need to work this much in order to be able to pay my bills/rent etc. but my mental health is in shambles and I really need a break. Is there a way to take a paid leave of absence? I really want to take a LOA but I need to have a source of income during it. Or is there any other thing I could do? I really like working here, but the amount of work is catching up with me and I don’t know how much longer I can do.
I close 5-6 nights a week. I can’t do it anymore. I’m an SSV and we don’t have a lot of closing SSVs. It’s just me and one other SSV, but that SSV doesn’t have a lot of night availability. I can’t do it. It’s getting harder for me to do my job and I feel like all the baristas can tell I’m worn out.

If it's you doing ALL the closing, that's not your problem. That's your managers problem, they should be looking to promote night people. You CANNOT do all of it.
My manager mentioned to me about taking a paid LOA for mental health? I don't know how to go about it, call sedgwick and ask, but I think you just need a therapists note. I'm not sure if you need a managers permission to do it since it's for a medical thing

You'll have to file for a medical LOA in order for it to be paid. Even then, it would only be 66% of your average pay. You can take a personal LOA (both of these LOAs you'd have to be benefits eligible in order to be approved) but that would be unpaid, however, you could use any sick or vacation hours for either.
