I have so many questions about LOA. Long story short my mom is in the hospital. Which is already stressful but I have to take care of my little brother who is autistic with an age delay. And with me and my dad working almost similar schedules he leave at 4am and gets home a 5pm. I work mid and pre closing so from 12-5 or sometimes till 8. My main thing is losing my health insurance since I have epilepsy and other conditions. So the main questions are
1. Do I loss my health insurance
2. What is the minimum amount of time is the leave.
3. Would I still be considered a partner for things like Spotify and discounts.

Six weeks
You have to have the money to pay your insurance. They will cancel it if it's not paid.
If the leave is approved then all benefits and perks remain.
This is NOT a SM or DM approval. You must have your mom's doc fill out the proper paperwork and get it turned in asap

From my own experience you do get your discount in the store, but things like Spotify will be turned off until you return. You can keep your insurance but have to pay for it out of pocket which can be a real struggle. I don't think there is a minimum time period for leave, or a maximum for that matter either. The people at Sedgwick are usually very kind and take time to help you sort things out during this period. I wish you and your family the best of luck!

Spotify is not turned off and you only have to pay YOUR contribution for health insurance (so whatever amount comes out of your paycheck has to be paid out of pocket)

If you don’t pay for it normally, you shouldn’t pay for it on leave. The only time you have to pay to keep a benefit on leave is if it’s something that normally comes out of your paycheck (such as health insurance)
I don’t specifically have experience with ASU but PCC should be able to answer any questions you have more clearly!

Did you have to do anything to keep your Spotify going? I am on leave now and have been a number of times and I always have to set everything back up when I return. It would be pretty cool to have right now :)

Thanks for everything! Thankfully, my mom is getting discharged this weekend. So I won't need to do the LOA.
