Don't mention a word of wanting to transfer to anyone until you've lined things up with your new SM and put the request in - limits the opportunity for your current SM to give you a pre-emptive write up.

I would also change your availability and stick to it. "Oh, you have me closing? I'm afraid that's outside when I'm availability, remember, so I won't be able to make it."
They don't need to know why or what you've got going on, just that your availability is a priority for you and a boundary you won't budge on. That could at least help with the mental health aspect.

Other than that, it sounds like you're the only one following standards. Consider reprint to partner resources or to your state health/ ethics body. That should get upper mgmt paying attention.

Should I request the transfer with my DM or just the new SM (or both)? You’re right, I’m sure if I put in a request now before I’d return In a month it would have to be in effect. I already have to drive so far out of the way for this store so it honestly would be better anyway just to get over to the one that’s 7 minutes away from me. Calling PRC sounds like a good idea as well, thanks!

Yeah I’m getting paid through sedgwick but for some reason they want to see if I want to use sick time which is confusing to me

I think they're required to ask. Sedgwick is an independent insurance company. You also only get 66.6% of your pay instead of the full pay like you do is sick days
