Typically the espresso is shaken with the ice and poured into the cup, topped with milk. I only add a little more ice if the espresso melts an unnecessary amount of ice and it becomes mostly milk.
Rule of thumb to avoid this though is fill the ice level with the sizes "ice" line

Cbs classic pumps 2-4-6 into shaker. Ice scoop matching the size (if you do it this way you don’t need to top off with ice), add shots 2-3-4, shake ten times. Pour into cup, top with 2%

It most definitely gets 2% milk by default you can ring one up to verify
